A subsidy health and safety inspection (SHSI) was conducted in conjunction with the code compliance inspection. Subsidy standards that are not covered in code were reviewed during this SHSI.

The center did not ensure at least every two years from the date of initial screen an updated Tuberculosis (TB) test screening is completed.

1. The record for staff #1 contained a Tuberculosis (TB) test that was dated for 3/24/2022. An updated tuberculosis (TB) test should have been completed on or before 3/24/2024.
2. The record for staff #3 contained a Tuberculosis (TB) test that was dated for 3/22/2022. An updated tuberculosis (TB) test should have been completed on or before 3/22/2024.
3. The record for staff #4 contained a Tuberculosis (TB) test that was dated for 7/27/2022. An updated tuberculosis (TB) test should have been completed on or before 7/27/2024.
