Violations

1
Standard 8VAC20-780-330-B
Based on observation, it was determined that there was insufficient resilient surfacing in front of the green slide on the older playground.
Evidence:
There were 4 divots in the resilient surface ( poured-in-place) 1 to 3 feet in front of the slide. The divots made the poured-in-place surface out of compliance with the minimum safety standards for resilient surfacing.
Plan of Correction: We will place the slide off limits to the children by using caution cones and signage and by training staff to ensure that they help block access by the children.
