During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 12/3/24. The inspector reviewed compliance in the areas listed above. There were 95 children present and 18 staff. The inspector reviewed 6 children?s records and 5 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
At the time of employment, the center must submit for a copy of the results of a criminal history record check from any state in which the individual has resided in the preceding five years.
The center had not submitted for the out of state criminal records check for staff A and staff B, Both resided in another state within the last five years. Staff A has been employed for two months and staff B has been employed for 1.5 months.
Plan of Correction: We were able to obtain the background checks during the inspection.

The center shall obtain a physical examination within 30 days of enrollment.
Child A enrolled 7 months ago and there was no physical examination on file.
