During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6/23/25. The on-site inspection began at 11:45 am and ended at 1:55 pm. The inspector reviewed compliance in the areas listed above. There were 70 children present and 15 staff. The inspector reviewed 5 children?s records and 6 staff records on-site.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

There were no immunization documentation on file for child A before attending, this information was overdue by one day.
Plan of Correction: We will obtain the immunization record.
Standard 8VAC20-780-40-M
Repeat Violation:
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. This list shall be dated and kept confidential in each room or area where children are present.

Child A has an dietary restriction to dairy, however this information was not listed on the allergy/sensitivity/dietary restriction, today was the child's first day of attendance.

Child B has an allergy to egg, oats and beans however the allergy/sensitivity/dietary restriction listed dated 6/19/25 has these foods as dietary restriction and not an allergy.
Plan of Correction: We will update the allergy/sensitivity/dietary restriction list.
Standard 8VAC20-780-60-A-8
A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

There was no allergy care plan on file for child B's allergy to egg, oats and beans.
Plan of Correction: We will obtain the allergy care plan.
