Inspector Notes
An unannounced monitoring inspection was conducted on 09/25/2023 and concluded with an electronic record review on 09/28/2023. There were 11 children, ages 3 years- 4 years, and 2 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during arrival from field trip and nap time. The inspector discussed the following with provider: physicals. A total of 5 children?s records, and 2 staff records were reviewed. The inspector arrived for the inspection at 11:15 am and departed at 1:30 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

1Evidence: The inspector observed the children and staff returning to the center from a field trip. The schedule of the trip?s events and location was not posted. Staff 1 confirmed they did not post the required information.
