Inspector Notes

An unannounced monitoring inspection was conducted on 04/15/2024 and concluded with an electronic record review on 04/18/2024. There were 18 children, ages 4 years - 5 years, and 2 staff members providing direct care and supervision present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies, and nutrition. The children were observed during mealtime, teacher-directed activities, and outdoor activities. The inspector discussed the following with the provider: sanitizing, medication authorization and immunization religious exemption. A total of 5 children?s records, 2 staff records, and 1 medication were reviewed. The inspector arrived for the inspection at approximately 9:00 am and departed at approximately 10:00 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 8VAC20-780-130-A
Based on record review, the center failed to obtain documentation that each child has received the immunization required by the State Board of Health before the child can attend the center.

Evidence: The record for Child 4 documented the first date of attendance as 08/23/2023. The immunization record contained the statement ?doesn?t vaccinate.?
Plan of Correction: Staff will discuss options (medical exemption or religious exemption) with parent and obtain proper documentation.
Standard 8VAC20-780-80-A
Based on observation and discussion with staff, the center failed to ensure that for each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence: At approximately 9:05 am on 04/15/2024, there was no daily attendance for the group of 18 children in care. Staff 2 confirmed the attendance had not been done and that the children began to arrive at approximately 7:15am. The daily attendance record did not document the arrival of each child as it occurred.
Plan of Correction: Staff will document arrival and departure as it occurs.
