Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was conducted on August 17, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were four children present, one being a household member, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of one child record and one caregiver record were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-180-A
Based on review and interview, provider did not ensure she has an updated TB test or screening every two years.

Evidence: The record for provider had documentation of a TB test/screening dated July 11, 2018 which is more than two years old. Provider confirmed this is the most recent TB test/screening.
Plan of Correction: Provider will go to doctor's office to have the test completed.
Standard 22VAC40-111-60-B
Based on review and interview, provider did not ensure a child record had documentation of required information.

Evidence: The record for child #1 (start date November 3, 2019) did not have documentation of insurance policy number. Provider confirmed information was not documented.
Plan of Correction: Provider will ask parent for policy number today.
