Inspector Notes

A monitoring inspection was conducted on February 28, 2023. There were five children present, with one being a household member, with one caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and one caregiver record were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on observation, provider did not ensure 2 of 2 child records has a physical examination prior to child's attendance of within 30 days after beginning attendance.

Evidence: The records for child #1 (start date 11/25/2022) and child #2 (start date 11/25/2022) did not have a physical examination. Provider stated parent provided the physicals but she could not find it.
Plan of Correction: Provider will look for copies provided by parent. If physicals cannot be located, she will request a copy again from the parent.
Standard 8VAC20-800-50-A
Based on interview and review, provider did not ensure there is a written record of children in attendance each day.

Evidence: Provider stated she did not have attendance documented for this day. The attendance record did not show attendance documented since 2/16/2023.
Plan of Correction: Attendance will be documented today and each day.
Standard 8VAC20-800-60-B
Based on review, provider did not ensure 2 of 2 child records have documentation of required information.

Evidence: The records for child #1 (start date 11/25/2022) and child #2 (start date 11/25/2022) did not have documentation of emergency contact addresses.
Plan of Correction: Provider will request information from the parents.
