Inspector Notes

A monitoring inspection was conducted on August 21, 2023 from approximately 10:55 am to 11:50 am. Five children currently enrolled were present with one provider. Children were observed napping. Provider record and six child records were reviewed. Violations are noted on the violation notice.

Repeat Violation
Based on review, provider did not ensure 3 of 6 child records have documentation of a physical prior to beginning attendance or within 30 days after the first day of attendance.

Evidence: The records for child #2 (start date 11/25/2022), child #3 (start date 11/25/2022) and child #4 (start date 6/24/2021) did not have a physical.
Plan of Correction: Physicals will be located and placed in files.

Repeat Violation
Based on observation and interview, provider did not ensure there is written record of children in attendance each day.

Evidence: There was five children enrolled present during inspection. Provider stated she had not completed the attendance sheet for this date.
Plan of Correction: Attendance will be documented today. In the future, attendance will be documented as children arrive and depart the family day home.

Based on interview, provider did not ensure the menu for the current one-week period is posted or given to parents.

Evidence: Provider stated she has not completed and posted the menu for this week.
