Inspector Notes

An unannounced monitoring inspection was conducted on June 10, 2024 from approximately 12:55 pm to 2:30 pm. Provider was present with four children enrolled and one household member child. Children were observed napping. Licensing Inspector reviewed standards related to recordkeeping, supervision, activities, napping, medication, emergency supplies and required postings. One caregiver, one household member and four child records were reviewed. Violations found are noted on the violation notice.

Based on review, provider did not ensure 4 of 4 child records have documentation of required information.

Evidence: The following was found during record review:
1. The record for child #1 had a most recent review of the child's emergency contact information dated 1/12/2022 which is more than a year old.
2. The record for child #2 had a most recent review of the children's emergency contact information dated 1/20/2023 which is more than a year old.
3. The record for child #3 (start date 8/21/2023) did not have a physical examination.
4. The record for child #4 (start date 8/21/2023) did not have insurance plan number.
