Inspector Notes

An unannounced monitoring inspection was started in the center on December 6, 2023 from 1:35 PM until 4:20 PM and concluded on December 13, 2023. At the time of entrance, 86 children were in care with 16 staff members supervising children. The sample size consisted of 9 children?s records, 9 staff records and 7 medications. Children were observed during story time, playing in centers to include home living, blocks and dramatic.
The playgrounds resilient surfacing was not inspected due to inclement weather.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and licensee interviews, the licensee did not ensure to a obtain the results out of state criminal records check (CRC) and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence: Staff records were reviewed and 2 out of 9 did not have out of state background checks as required,
1) Staff #1?s record did not include the out of state CRC or the CPS check for Michigan where they had lived within the last five years. Staff #1?s first date of employment was 8-21-2023.
2) Staff #9?s record did not include the out of state CRC for Massachusetts where they had lived within the last five years. Staff #9?s first date of employment was 8-1-2023.
