Inspector Notes

An unannounced complaint inspection was initiated on March 18. 2024 and completed on April 22, 2024 regarding concerns of administration, physical plant, programs, special care provisions/emergencies and staffing and supervision. During the inspection staff to child interactions and ratios were observed, naptime routine/ procedure reviewed, interviews with center staff, a review of center's records/documentation and observation of the classroom in which the incident occurred.

The preponderance of the evidence gathered during the investigation does NOT support the allegation related to physical plant, programs, staffing and supervision; therefore, the complaint is determined to be NOT valid.

However, the information gathered during the inspection determined violations with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on center documentation review and staff interviews, the licensee did not ensure the center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence that includes all required information.

Standard 8VAC20-780-80-B-2
Based on center documentation review and staff interviews, the licensee did not ensure to inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Evidence: Child #1 was injured at the center on June 23, 2023 and the injury required outside medical treatment (stitches) and the department was not notified of the injury.
