Inspector Notes

An unannounced monitoring inspection was initiated on 11/24/2025 and completed on 12/2/2025 in response to a self-report received by the licensing office on 10/23/2025 relating to Behavior Guidance and Forbidden Actions. The inspector reviewed 2 staff records on-site on 11/24/2025. This inspection included document review, and interviews.

The information gathered during the inspection determined no violations with applicable standards or law. No violations were issue
