Inspector Notes

An unannounced renewal inspection was conducted on 2/3/2023. There were 56 children, ages 4 months-4years, 11 staff and 4 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications for children were observed during the inspection. A total of 4 children?s records, 5 staff records and 3 medications were reviewed. The children were observed during circle time, center time and lunch. The inspector arrived for the inspection at 10:50am and departed the center at 12:15pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on observation, the center failed to ensure that the required nonabsorbent surface for diapering was used.

Evidence: The changing pad in the infant room was separated at the seam along the middle front edge approximately 1 ? inches and the padding was exposed.
Plan of Correction: Provider will replace diapering surface.

Based on the review of four children?s records, the center failed to ensure that the separate record for each child enrolled contains all of the elements as required by the standards.

Evidence: The record for Child #2 did not contain the following information: first date of attendance.
