During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/14/25 and completed on 10/14/25, as a part of the licensure period. The on-site inspection began at 9:45am and ended at 10:50am. The inspector reviewed compliance in the areas listed above. There were seven children present and two caregivers; the point total was thirteen. The inspector reviewed four children?s records and two caregiver records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Provider did not obtain a completed national criminal background check for Caregiver #2 prior to the employee's first day of employment. The criminal background check results were dated three days after employment at the day care.
Plan of Correction: Will ensure to obtain a completed national criminal background check prior to the employee's first day of employment going forward.

The provider must request a search of the central registry prior to the employee's first day of employment.

Caregiver #2's central registry search was requested two days after the first day of employment.
Plan of Correction: Will ensure to request the central registry background check prior to employment in the future.
Standard 8VAC20-800-120-B
Assistants and substitute providers' records must contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Caregiver #2's references were checked after employment. The first reference was contacted two days after hire, and the second reference was contacted six days after hire.
Plan of Correction: Will ensure to check two references prior to hire going forward.
