Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A monitoring inspection was initiated and concluded on 10/4/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 4 children present (12 points), ranging in ages from 5months to 2years, with the provider supervising. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 children's records and 1 caregiver record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-111-60-A
Based on a review of administrative records, it was determined that the provider has not maintained an up-to-date record at the family day home for each enrolled child.
Evidence:
The provider does not have an enrollment record on site for one child present and in care in today.
Plan of Correction: I gave her file to the parent during drop off this morning to update some information. The parent is returning the file at pick up later this afternoon.
Standard 22VAC40-111-60-B
Based on a review of children's records, it was determined that one record does not contain all required information.
Evidence:
The record for child A does not include the provider's signature indicating that she viewed the child's original proof of identification.
Plan of Correction: I saw her birth certificate. I will sign the form and update the file.
