Inspector Notes

An unannounced monitoring inspection was conducted today between 10:00 am and 11:20 am. There were no children present. A complete inspection in the areas of administration, physical plant, nutrition, fire drill log, and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-60-B
Based on record review, it was determined a child's record was incomplete.

Evidence:
Child #1 did not have emergency contacts addresses
Plan of Correction: I will have the parent provide the addresses upon their return from vacation.
Standard 8VAC20-800-830-D
Based on record review, emergency evacuation records were not maintained.

Evidence:
Emergency evacuation record was missing the documentation for the month of June. Provider stated drills were conducted but forgot to document.
Plan of Correction: I will document all drills.
Standard 8VAC20-800-90-C
Based on record review, it was determined documentation of additional immunizations was not obtained every 6 months for children under the age of 2 years.

Evidence: Child #1 (DOB 9/4/2020) record did not have additional immunizations. Last update was 09/02/2021.
Plan of Correction: I will ask parents to bring updated immunization record upon return from vacation.
