Inspector Notes
An unannounced renewal inspection was conducted on January 24, 2023 between 9:10am and 11:05am. There were 4 children present with one (1) caregiver supervising for a total of 12 points within the supervision guidelines. The children were observed being fed, diapering, and playing with various age-appropriate toys. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas administration, physical plant, supervision, programing, fire drill log, menu, attendance and emergency procedures. A total of 4 child records, caregiver and household member records were reviewed. No medication on sight at the time of inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the programs.

7Evidence: Household member #1 did not have documentation of an updated central registry results (expired 2/13/22).
Evidence:
1.Child #2 (date of enrollment 8/1/22) did not have documentation of a physical at the time of inspection (1/24/23)
2.Child #3 (date of enrollment 9/1/22) physical is dated 1/3/23.
Evidence: The record for Child #1 did not have written authorization for emergency medical care.
Evidence:
1.There was no documentation that a shelter-in-place was practiced during the year.
2.Provider acknowledged the shelter-in-place was not practiced in 2022.
Evidence:
1.Child #2 immunization record is dated 1/3/23 and start date was 9/1/22.

1. The menu was not posted.
2. The Provider acknowledged she did not have a menu for the week.
