Inspector Notes

A monitoring inspection was initiated on January 18, 203 and concluded on January 18, 2023. There were 2 children, totaling 4 points, present directly supervised by 1 staff. The children were observed engaged with puzzles with and without assistance, indoor play and handwashing.

Limited monitoring of child care requirements occurred during today?s visit. Licensing inspector monitored supervision of children, physical plant, staff/child ratios, adequate and approved space, programming, and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on children records review, the provider did not obtain within seven business days of the child's first day of attendance at the family day home, the provider shall obtain from the parent proof of age and identity for enrolled child.
Evidence:
1. Zeeshan P DOB (11/12/2012) did not have proof of age and identity in record available for review during the inspection.
Plan of Correction: I will get form parent today.
