Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, documentation review, interview, and a virtual tour of the program.

A monitoring inspection was initiated on 2/10/22 and concluded on 2/15/22. The director was contacted by telephone and a virtual inspection was conducted. There were 33 children present, ranging in age from six-months to five-years-old, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, medication, special care and emergencies, and nutrition. A total of three child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with an applicable standard and a violation was documented on the violation notice issued to the program.

Based on documentation review and interview, the center did not ensure that they obtained written authorization from the parent for medication. Evidence: Two emergency medications were on site for Child A on 2/10/22. There was no written authorization from the parent on site.
Plan of Correction: The missing signature for emergency medication consent was corrected the same day of the inspection. Future emergency medication will still be documented using the MAT Medication Consent Form. In the future, the Director/Administrator will highlight locations on the form where (parent & doctor) signatures are required as for the instructions. This will help ensure required signatures are included and not missed. In addition the Director and Administration have reviewed the forms to ensure they are filled out properly and completely.
