During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6/12/25, as a part of the provisional licensure period. The on-site inspection began at 2:50pm and ended at 3:35pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and six staff. The inspector reviewed two children?s records and one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.

The center is required to obtain a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

The record for Child B did not contain a physical. Child B was originally enrolled more than 21 months ago.
Plan of Correction: Will be receiving a new physical on 6/23/23.
Standard 8VAC20-780-60-A
(Repeat Violation) Child records shall contain all of the required information.

The record for Child B did not contain the name, address, and phone numbers of two designated people to call in an emergency if a parent cannot be reached.
