Playground equipment with moving parts or climbing apparatus is required to have a fall zone surrounding the equipment with resilient surfacing that meets the requirements for depth for the resilient surfacing material.
On 1/21/26, there was less than three inches of pea gravel at the front of the yellow bus climber and the grey rock climber. There was less than six inches of pea gravel around the sides and back of the two climbers. Pea gravel requires a depth of six inches to be considered resilient.

Plan of Correction: We are removing the climbing Apparatus and moving around the Rocks to meet standards. We are also moving the bus to correct this.

The center is required to ensure that their procedures for administering medication include methods to prevent the use of outdated medication.
On 1/21/26 an over-the-counter medication was observed onsite for Child F that expired in June 2025. Staff interview revealed the center was unaware that the medication was expired.

Plan of Correction: Medication was removed and replaced with new in dated medication. All staff was updated with new exp date along with medication signs in each room.
