Standard 8VAC20-780-40-E
The center is required to ensure that the center?s activities, services, and facilities are maintained in compliance with these standards, the center?s own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

The center?s safe sleep policies state that infants who fall asleep in a space other than their crib would be moved to a crib as soon as possible. On 2/17/26 at approximately 11:19am, a 12-month-old infant was observed sleeping in their car seat in the classroom. Interview revealed the infant had arrived at approximately 10:14am. The infant was not removed from their car seat until approximately 11:26am.
Plan of Correction: We provided that staff with the standards of sleep practices also provided more training on how to transition sleeping children to cots/cribs.
Standard 8VAC20-780-570-B
Bottle fed infants who cannot hold their own bottles are required to be held when fed. Bottles are not allowed to be propped.

On 2/17/26 an approximately 4.5-month-old infant was observed sitting in an infant seat with a blanket folded on their lap and chest with their bottle propped on the blanket.
Plan of Correction: We went over that state feeding standards with each staff member in the infant class.

The center is required to maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs.

On 2/17/26, at approximately 11:19am, six infants were observed present in the Infant classroom. The attendance record contained documentation of only four infants present.
Plan of Correction: Reinforcing the regulations on attendance and making sure everyone has the lead teacher training to understand the necessary policies.
