Standard 22.1-289.011-F
The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 1/7/2026, was not posted in a conspicuous place on the premises.
Plan of Correction: Corrected during the inspection. Going forward inspection results will be posted as soon as possible except for the supplemental page which shall remain confidential.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in safe conditions. The green ground covering on the playground is coming up creating a trip hazard. There are two holes in the wall of the bathroom used by children in care. The holes are within reach of children and large enough for a child?s finger to fit in. In the infant room there is peel paint within reach of one crib when occupied. There is one crib sheet that is loose fitting in a crib assigned to a child in care. The loose-fitting sheet poses an entanglement hazard.
Plan of Correction: Center management will remove Astro-turf on the step leading the playground and replace with a covering that does not pose a trip hazard.

Children under 10 years of age always shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom. A three-year-old child was allowed to go the rest room 2 classrooms away from where the staff could see and or hear the child.
Plan of Correction: All staff will be retained and reminded that children may go to the restroom supervised. A staff person will always be close enough to hear the child in the restroom.

When centers choose to provide meals or snacks, they will follow the most recent, age-appropriate nutritional
requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA). The afternoon snack served to children today did not follow USDA and consisted of fruit juice cocktail and powdered doughnut.
Plan of Correction: Going forward center management will ensure that all meals and snacks served to children meet USDA requirements.

No child shall be allowed to drink or eat while walking around. Children in the toddler room were observed walking around the room while drinking from sippy cups.
Plan of Correction: Center management will remind and retrain all staff that children must always be sitting while eating or drinking.
