Inspector Notes

An unannounced monitoring inspection was conducted on April 17, 2023 from 10:25 a.m. to 11:50 p.m. There were 45 children present, ranging in ages from 3 years old to six years old with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Based on review of records and interview, the center failed to ensure that each child had a physical examination before the child?s first day of attendance or within 30 days of attendance.
Evidence:
1. Child #2?s start date was 1/3/2023. The child?s record did not contain evidence of a physical examination. Staff #5 confirmed that Child #2?s record did not contain evidence of a physical examination.
2. Child #4?s start date was 10/28/2021. The child?s record did not contain evidence of a physical examination. Staff #5 confirmed that Child #4?s record did not contain evidence of a physical examination.
Plan of Correction: The families of each child have been asked to supply the proper proof of physical examination by April 28, 2023. Moving forward, the admin assistant will ensure and document that each student's file is complete, accurate, and with appropriate signatures.
Standard 8VAC20-780-150-B
Based on review of records and interview, the center failed to ensure that each immunization report was signed by a physician, his designee, or an official of the local health department.
Evidence:
1. The immunization report on file for Child #2 was not signed by a physician, his designee, or an official of the local health department.
2. Staff #5 confirmed that Child #2?s immunization records was not signed by a physician, his designee, or an official of the local health department.
Plan of Correction: On April 17, 2023 and moving forward, the admin assistant will ensure and document that each student's file is complete, accurate, and with appropriate signatures.
Standard 8VAC20-780-350-O
Based on observation and interview, the center failed to maintain the required ratio for balanced mixed age group of one staff member for every 14 children.
Evidence
1. The inspector observed the Primary Two classroom to have 26 children present with one staff member.
2. Staff #1 confirmed there were 26 children in the classroom and that they were they only staff member present.
Plan of Correction: Beginning April 18, 2023, At predetermined times each day, the office admin will go to each classroom to allow both teachers to take a break, one at a time.
