Inspector Notes

An unannounced monitoring inspection was conducted on 11/15/2023 from 11:30 a.m. to 1:00 p.m. There were 34 children present, ranging in ages from three to four years old with six staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the center failed to complete orientation training with each staff member no later than seven days of the date of assuming job responsibilities.
Evidence:
1. Staff #1?s start date was 07/01/2023. There was no documentation regarding orientation on file for Staff #1. Staff #5 confirmed there was no documentation regarding completed orientation on file for Staff #1.
Plan of Correction: Conduct orientation immediately.

Based on measurement, the center failed to ensure that when playground equipment is provided, resilient surfacing complies with the minimum safety standards.
Evidence:
1. The mulch depth on the playground measure three to five inches in varying spots.
Plan of Correction: Mulch has been ordered.
Standard 8VAC20-780-60-A
Based on review of records and interview, the center failed to maintain for each child a separate record with the required information.
Evidence:
1. Child #1?s record was missing the phone number of the place of employment for two parents.
Plan of Correction: Parents were unavailable until 11/27/23 when we emailed them for updated info.
