During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on August 13, 2025 and completed on October 16, 2025 in response to a self-report received by the licensing office on August 12, 2025 relating to staffing and supervision. The inspector reviewed 1 child record, and 2 staff records on site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 23, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

During outdoor playground time, Child #1 (age five years old) remained inside the classroom alone for approximately 24 minutes while the rest of the class went outside. Staff were unaware that Child #1 had been left inside until the group returned from the playground. It was determined that staff did not have an accurate attendance count of the children present during this time.
Plan of Correction: Responsible staff members received appropriate disciplinary action. All staff were retrained on keeping accurate attendance and child supervision. All staff received training to ensure transitioning children are accounted for given the potential that children may hide before exiting the classroom or playground.
Standard 8VAC20-780-340-F
Children under 10 years of age always shall be within actual sight and sound supervision of the staff.
On August 12, 2025, Child#1 (age five years old) was left inside the classroom alone without actual sight and sound supervision of the staff for approximately 24 minutes.
Plan of Correction: Responsible staff members received appropriate disciplinary action. All staff were retrained on keeping accurate attendance and child supervision. All staff received training to ensure transitioning children are accounted for given the potential that children may hide before exiting the classroom or playground.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

On August 12, 2025, one child's arrival in the preschool group was not recorded by the staff during morning check-in. As a result, the attendance record and threshold count for the group were inaccurate for part of the day.
