Inspector Notes
An unannounced inspection was initiated on 5/2/2025 and completed on 6/9/2025 in response to a complaint received by the licensing office on 3/4/2025 relating to allegations of Administration, Physical Plant, Supervision, Programs and Special Services. The inspector reviewed 1 children?s record and 3 staff records on-site on 5/2/2025. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
