Inspector Notes

A monitoring inspection was completed on October 18, 2023 at the center from 10:09 am until 12:14 pm. There were 100 children present with 22 staff supervising. The inspector completed observations of the children in care participating in centers, interacting with peers and staff, completing bathroom breaks/handwashing, and engaged in outdoor activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of child records and 6 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of documentation and interview, the center did not ensure to obtain an annual fire inspection.

Evidence: During the inspection, the last fire inspection documented was dated 01/24/22. Interview with staff confirmed documentation available for review.

Based on observation, the center did not ensure all areas inside and outside were maintained in a clean, safe, and operable condition.

Evidence: There were two classroom sinks observed with visible cracked paint on the bottom, and rusting.

Based on observation, the center did not ensure all hazardous substance were kept in a locked place using a safe locking method that prevents access by children.

Evidence: The maintenance closet was observed open and unlock containing multiple disinfectant and glass cleaners, and primer with the labels "keep out of reach of children" , "danger," "caution" and "warning".
