Inspector Notes
An unannounced inspection was initiated on 5/20/2024 in response to a self report that was received by the licensing department on 5/9/2024 regarding an allegation in the area of supervision. The inspection was initiated on 5/20/2024 and completed on 5/29/2024. On 05/20/2024, two inspectors were on site from approximately 9:07 am to 11:22 am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. An additional interview was conducted remotely.
The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the self report is determined to be valid. See the violation notice for more details.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. 2Evidence:
1. Video footage was reviewed from 5/7/2024. In the video, child #1 (age 4) and child #2 (age 4) were observed sitting behind a shelf in the classroom. For approximately two minutes, the children took turns exposing and inappropriately touching each other with their hands and mouths. In the video, staff #1 is observed sitting in a chair and looking at her phone during the incident. This exchange took place five times before staff #1 (date of employment: 1/30/2023) intervened.
Evidence:
1. On May 7, 2024, two children (aged 4) inappropriately touched each other for approximately two minutes while positioned behind a shelf and out of staff #1's full sight. In the video, staff #1 is observed sitting in a chair and looking at her phone during the incident.
2. In an interview, staff #1 acknowledged that she was using her phone and did not have full visibility of the two children.
