Inspector Notes
An unannounced, on-site renewal inspection was initiated on 7/15/2024 and completed on 7/15/2024. The on-site inspection began at 10:45 and ended at 12:15. The inspector reviewed compliance in the areas listed above. There were 12children present and 2 caregivers; the point total was 31. The inspector reviewed 5 children?s records and 3 staff records as well as 3 household member records. This inspection included

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
The plan of correction has been entered according to the plan the provider indicated.
Violations
5Evidence: Based on the review of staff records, the assistant, staff 2 is missing the finger print check. DOH 5/30/24.
Evidence: The assistant is missing a second reference.
Evidence: Reviewed 5 children's records, child 2 and 3 are missing the information about the providers emergency plan and child 2 is missing the liability insurance form.
Evidence: The provider has an EpiPen in the first aid kit it kept on a shelf inaccessible to the children. The first aid kit is not locked, a padlock is holding two eye screws together but is not hooked through the eye screw.
Evidence: Based on the review of the emergency preparedness plan the plan is missing a updated signature for the last two years.
