Inspector Notes

An unannounced, on-site renewal inspection was initiated on 7/15/2024 and completed on 7/15/2024. The on-site inspection began at 10:45 and ended at 12:15. The inspector reviewed compliance in the areas listed above. There were 12children present and 2 caregivers; the point total was 31. The inspector reviewed 5 children?s records and 3 staff records as well as 3 household member records. This inspection included

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
The plan of correction has been entered according to the plan the provider indicated.

Standard 22.1-289.035-B-2
Based on the review of staff records, the provider failed to get finger print checks done for one staff.
Evidence: Based on the review of staff records, the assistant, staff 2 is missing the finger print check. DOH 5/30/24.
Plan of Correction: My assistant has to go to the embassy to get a copy of her passport. she will get that on 7/29/24v and then we will get her background checks.
Standard 8VAC20-800-120-B
Based on the review of staff records, the provider failed to get some required information for an assistant:
Evidence: The assistant is missing a second reference.
Plan of Correction: I will ask for a second reference for my new assistant.
Standard 8VAC20-800-70-A
Based on the review of children's record, the provider failed to give some information to parents on the first day of attendance:
Evidence: Reviewed 5 children's records, child 2 and 3 are missing the information about the providers emergency plan and child 2 is missing the liability insurance form.
Plan of Correction: I will get that information to the parents
Standard 8VAC20-800-730-D
Repeat Violation.Based on the review of medication, the provider failed to keep medication in a locked space.
Evidence: The provider has an EpiPen in the first aid kit it kept on a shelf inaccessible to the children. The first aid kit is not locked, a padlock is holding two eye screws together but is not hooked through the eye screw.
Plan of Correction: Fixed during the inspection. I have a lock I can use with a key.

Based on the review of the emergency preparedness plan the provider failed to do the annual update.
Evidence: Based on the review of the emergency preparedness plan the plan is missing a updated signature for the last two years.
Plan of Correction: Fixed during the inspection, I will make sure I review and sign annually.
