Inspector Notes

An unannounced complaint inspection was conducted in response to a complaint that was received by the department on 09/22/2021 regarding allegations in the areas of forbidden action and behavioral guidance. The inspection was initiated on 09/28/2021 and completed on 10/04/2021. On 09/28/2021, the inspector was on site from 9:58 am to 10:30 am. Observations were made in the center. Documentation was reviewed remotely and telephone interviews are conducted.

The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. There are no violations resulting from this inspection.
