Inspector Notes
An unannounced monitoring inspection was initiated on 09/25/2024 and completed on 10/10/2024 with a local agency, in response to a self-report received by the licensing office on 09/18/2024 relating to forbidden action of a child. The inspector reviewed three children?s records and three staff records on-site. This inspection included document review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/04/2024. A POC submitted after this date will not appear on the public website.
Violations
2In the center?s personnel policies and procedures, it states that staff, consultants, contractors, and volunteers will implement positive strategies to support children's well-being and prevent and address challenging behavior. On 09/17/2024, staff #3 pulled the arm of a child that was stuck in their chair and slapped and flicked the hands of children to correct their behavior.
completed. The accused staff was placed on leave pending the results of the CPS investigation. The agency conducted an all-staff training that focused on classroom management skills and provided staff with additional support.
On 09/17/2024, child #1 and child #2 were playing with food during lunch time when staff #3 slapped and flicked the hands of the children to correct behavior. Another incident on 09/17/2024, child #3 was struggling to get out from the table due to their leg being stuck on the chair. Staff #3 yanked and pulled the arm of the child to remove child from the seat. Child #3 cried due to being pulled from the chair.
The agency conducted an all-staff training that focused on classroom

The accused staff is receiving additional training and coaching to help strengthen their classroom management and teaching strategies.
