Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 04/07/2026 and completed on 04/13/2026. The on-site inspection began at 9:37am and ended at 11:21am. The inspector reviewed compliance in the areas listed above. There were 40 children present and seven staff. The inspector reviewed five children?s records and five staff records electronically on 04/13/2026. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1Medication for child #8 was not labeled with the child's name, the name of the medication, the dosage amount, and the time of times to be given.
