Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 11:50am and 1:10pm. There were 5 classrooms observed with a total of 42 children with 7 staff within the supervision guidelines. The children were observed watching an interactive STEM video and napping. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were reviewed during this inspection. The violations from this inspection can be found on the violation notice.

Based on interview, the child day program has not ensured that the building was equipped with at least one carbon monoxide detector.
Evidence: When asked, the program director confirmed that a carbon monoxide detector had not yet been installed in the building.
Plan of Correction: A carbon monoxide detector has been installed however building runs solely on electric.
Standard 8VAC20-780-130-A
Based on record review, the center has not obtained documentation that each child has received their immunizations before the child could attend the center.
Evidence: Child #2 (date of enrollment: 8/22/2022) record does not contain documentation of immunizations.
Plan of Correction: Child #2 has an updated immunization on file at the center.
Standard 8VAC20-780-130-E
Based on record review, the center has not obtained documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #1 (13 months) most recent documented immunizations are dated when the child was approximately 4 months old.
Plan of Correction: Child #1 has an updated immunization on file at the center.

Based on record review, the center has not obtained documentation that each child has a physical exam by a physician within the first 30 days of enrollment.
