Inspector Notes

An unannounced, on-site monitoring inspection was conducted 7/25/2024 from 9:35am - 2:10pm. The inspector reviewed compliance in the areas listed above. There were 69 children present and 12 staff. The inspector reviewed 6 children?s records and 4 staff records on-site. This inspection included document review (i.e. Injury logs, policies and procedures, emergency drill logs, training logs, medication administration logs, authorization and permission forms), interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center, outside, was not maintained in a safe and operable condition.

Evidence:
1) The garden boxes on the preschool playground has chipped and peeling paint.
2) The preschool and school age playground has depleted mulch along the cemented surface, causing a tripping hazard.
3) On the JK playground, there are sections of the wooden fence that are rotted.
4) On the preschool playground, the corner section of the wooden fence has a partially attached and rotted baseboard and screws that are coming loose, making the fence unstable.

Standard 8VAC20-780-40-M
Based on record review and interview, the center did not maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies.

Evidence:
Child #1 (DOE: 6/17/2024) has a confirmed food allergy that was not documented on the allergy chart posted in the classrooms.
