During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was conducted on 5/30/2025. The on-site inspection began at 10:15am and ended at 12:20pm. There were 47 children and 12 staff present during the inspection. The inspector reviewed 5 child records and 3 staff records on-site. This inspection included document review, interviews, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which is 6/12/2025 . A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-140-A
Each child shall have a physical examination by or under the direction of a physician before the childs attendance or within 30 days after the first day of attendance. The current form required by the Virginia Department of Health shall be used to report the results of the examination and shall be signed by a physician.

A well visit summary was obtained for Child #1 prior to enrollment but it did not contain all required components or a physicians signature. A complete physical examination was not obtained until approximately 6 months after the first date of attendance.
Plan of Correction: Child #1 had a current physical examination at the time of the licensing inspection with all required components.
Standard 8VAC20-780-260-B
Annual approval from the health department shall be provided for meeting reuirements for water supply, sewage disposal system, and food service (if applicable).

The most recent health inspection, conducted in August 2024, did not include the sewage disposal system.
Plan of Correction: The health department has been contacted to schedule an inspection of the sewage disposal system.
