Inspector Notes

An unannounced monitoring inspection was conducted on June 21, 2022. The sample size consisted of 5 children's records and 5 staff records. Children were observed outside playing on the playground, participating in chalk activites , children in the classroom participating in multiple station activiities. The inspection began at approximately 9:30am and ended at approximately 12:04 pm. Areas of non-compliance are identified on the violation notice.

Based on observation, playground surfacing measurements taken, the program did not ensure that adequate resilient surfacing (depth of six inches) was maintained under/surrounding the outdoor playground equipment.

Evidence: 1. Observation and resilient surfacing measurements, taken on this date, determined that the resilient surfacing surrounding the 2 playground composites that include slides and climbing apparatus and the playground stepping pods were approximatley 2 inches depth.
