Inspector Notes

An unannounced monitoring inspection was conducted on 03/31/2023 There were 144 children
present at this time with 28 staff members. Children were observed participating in a outdoor activities, infants being fed and tummy time, prepare for activity and activity stations . Seven staff record and five
children's records were reviewed. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, medication procedures, emergency evacuation and shelter-in-place drill
documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the licensee did not ensure the child day center obtained a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:

The record for staff #2, date of hire 6/23/2022 did not include the results of a criminal record check for the state of New Hampshire, where he had lived within the last 5 years. The criminal record was not conducted prior to employment as required.

The record for staff #4, date of hire 9/19/2022 did not include the results of a criminial history record check and central registry record check for the state of Massachuseets, where he had lived within the last 5 years.

Standard 8VAC20-780-160-A
Based on staff interview and record review, the licensee did not ensure each staff member submits documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: Staff #1 (start date 02/17/2023) the TB provided was 03/15/2023 . Documentation was not completed with in last 30 calendar days of the date of employment.

Staff # 5 ( start date 1/16/2023) the TB provided was 01/18/2023. Documentation was not completed with in last 30 calendar days of the date of employment.
