Inspector Notes
The licensing inspector conducted an self report in regards to outside medical treatment that was received by the licensing department on January 9, 2024 . The inspection was initiated on January 18, 2024. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, licensee did not ensure the climbing portions of indoor slides and climbing equipment over 18 inches shall not be over bare flooring.
Evidence:
In the School age area, there was climbing equipment the highest point was 2 feet. The equipment was over laminate floor.

Plan of Correction: Black mats were ordered immeidatley January 10, 2024. And additional three packet of mats were order January 18, 2024 to cover all area around the equipment.
