Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #2 TB was dated eight months prior to date of employment and Staff #5 TB was dated forty-two days prior to date of employment.
Plan of Correction: Staff #2 had a valid negative TB test however transferred to The Kensington School from another local center, and we did not update the TB test within the 30 day. Staff member updated TB test on 12/1/25 (negative result)

Cribs shall be used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat. Two children in the infant room, under 12 months of age, were placed in swings for their nap.
