Inspector Notes
An unannounced monitoring inspection was conducted on October 24, 2012. There were 94 children present at arrival to the center with twelve staff members. Additional staff were on site for administrative and support purposes. The children were observed napping and playing freely in the classrooms. Licensing inspector reviewed supervision, activities, equipment, napping procedures, diapering and hand washing procedures, medication, children and staff records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings.

5Evidence: During the inspection conducted on October 24, 2012, the record for staff #9 (start date 7/27/2012) does not have a TB test.
Evidence: During the inspection conducted on October 24, 2012, the licensing inspector observed hand sanitizer in four out of eight classrooms that were labeled "warning... keep out of reach of children".
Evidence:
1. During the inspection conducted on October 24, 2012, the licensing inspector observed that the center did not meet the required staff-to-children ratio (one staff member to every four infants) in the 5-11month old room. There were two staff present with 11 children in care.
2. On the same date, the licensing inspector observed that there was one staff present with 12 napping children in the 12-18 month old room.
Evidence: During the inspection conducted on October 24, 2012, the licensing inspector observed diaper cream ointments stored in a basket under the diaper table in the 5 month-11 month classroom. The ointments were accessible to children.
Evidence:
1. During the inspection conducted on October 24, 2012, five out of five injury reports were missing the time that the parents were notified.
2. During the inspection conducted on October 24, 2012, three out of five injury reports were missing any future action to prevent recurrence of the injury.
