Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.
A monitoring inspection was initiated on 3/18/22 and concluded on 3/21/22. The director was contacted by telephone and a virtual inspection was conducted. There were 24 children present, ranging in ages from infants to five, with five staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on record review and interview, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:

1. The record of child 1 documents the start date as 11/15/21. The most current immunization record is dated as 12/10/21.
2. The director verified the 12/20/21 immunization record is the most current immunization the center has.
Plan of Correction: In the future no child will be allowed to attend the center until immunization documentation is obtained.
