During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and concluded on July 14, 2025. The on-site inspection began at 10:17 a.m. and ended at 11:40 a.m. The inspector reviewed compliance in the areas listed above. There were 22 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information on Safe Sleep for Infants was provided to the center.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 30, 2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There was a crack in the wall in the infant room and splintered wood and chipped paint on the deck.
Plan of Correction: Director has contacted a painter to come out and repair the crack in the wall which is in the infant room. His estimate time to this repair is no later than August 5, 2025. This person will also repair the splintered wood and chipped paint on the deck within that same time frame of August 5, 2025. I will send photos when the work is done.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Infant #1 (age 11 months) was observed asleep in a crib covered with a blanket up to the infant?s neck. Staff #3 reported that infant #1 was asleep for approximately 25 minutes.
Plan of Correction: All employees have received and read the form on safe sleep practice. They have signed and dated a staff acknowledgment form for safe sleep practice. In addition I have posted 2 information sheets on safe sleep for infants on the walls to be a reminder on this practice. These information sheets were provided from you on 7-22-25.
