Inspector Notes

An unannounced Monitoring inspection was conducted on November 30, 2022. Upon arrival at 2:30 PM there were 4 children ages 9 months - 3 years old and 1 staff person present. An additional 16 children and 1 staff person arrived during the inspection. The Licensing Inspector reviewed 3 children records and 2 staff records along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.031-A-3-a
Based on observations & discussion with staff, the Religiously Exempt Center failed to maintain required adult/child ratios based on the youngest child in care.

EVIDENCE:
Upon arrival at 2:30 PM there were 4 children ages 9 months - 3 years old under the care of 1 staff. During the inspection, an additional 16 school age children (ages 5-10 yrs old) and 1 staff person arrived bringing the total number of children present to 20 (ages 9 months-10 years old). There were 2 staff persons caring for the 20 children in one group. Required adult/child ratios of 1 staff per 4 children was not maintained.
Plan of Correction: Enrollment will be decreased until additional staff is hired.

Based on record review & discussion with staff, the Religiously Exempt center failed to ensure Fingerprint Background Check is completed before employment and every 5 years thereafter.

EVIDENCE:
Licensing Inspector reviewed 2 staff records.

Fingerprint Background Check for Staff #2 was originally completed 7/23/2017 and was not updated within 5 years.

Child Protective Services Background Check for Staff #1 was originally completed 10/3/2017 and was not updated within 5 years.
Plan of Correction: Staff will complete background checks within ten days. Documentation of findings will be maintained in the staff records.

EVIDENCE:
Center was not equipped with at least 1 carbon monoxide detector.
