An unannounced, on-site code compliance inspection was initiated on (date) and completed on (date). The on-site inspection began at (time) and ended at (time). The inspector reviewed compliance in the areas listed above. There were 6 children present with 2 staff. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable code sections, and violations are documented on the violation notice issued to the program.

Upon enrollment of a child the regulated child day program shall require that the person enrolling the child present the child day program with the proof of the child's identity and age.

Center did not obtain the proof of identity for Child #3 and Child #5.
Plan of Correction: Management will have parents submit proof of identity for each child.
