Inspector Notes

An unannounced renewal inspection was conducted on March 13, 2023 from 10:55 a.m. to 12:15 p.m. There were four children present, ranging in ages from 23 months to four years old with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and one staff record was reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of records and interview, the provider failed to obtain verification of age and identity of the child within seven business days of the first day of attendance.
Evidence:
1. Child #2?s first day of attendance was 10/03/2022. There was no verification of the identity and age of the child in Child #2?s record.
2. The provider confirmed that Child #2?s record did not contain verification of the identity and age of Child #2
