During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 6/4/25. The on-site inspection began at 10:30am and ended at 2:20pm. The inspector reviewed compliance in the areas listed above. There were 37 children present and nine staff. The inspector reviewed seven children?s records and four staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 6/18/25. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-E
Repeat Violation
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
The record for Child #1 (enrolled for one year) did not contain the additional immunizations, which is required for children under the age of two years.
Plan of Correction: Our center policy is sent out to parents in January and June of each year and
also updated at the start of the school year in September. We?ve created a printable checklist to ensure that as the updated shot records come in we are signing to confirm that we have received new records from parents.

Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition.
A toy sink in a toddler classroom was broken and not in operable condition. There was an insect nest inside of a playhouse on a toddler playground.
Plan of Correction: Staff have been informed and updated
of the requirements to remove/discard
any broken toys immediately as well as
the importance of checking inside the
playhouses during morning routine

Repeat Violation
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
In an infant room, cleaning supplies such as window cleaner, air fresheners, and disinfectant were stored in an unlocked closet.
Plan of Correction: Infant teachers were retrained
and reminded of the importance of
ensuring classroom closets are locked
at all times even in an infant class
with no walking children.
Standard 8VAC20-780-350-M
One staff member shall not supervise more than one room or area during rest time.
Staff #1 (employed for 11 years) was observed supervising two classroom alone during rest time.
Plan of Correction: Staff have been advised and retrained on class supervision now that our 3-4
are split into two classrooms. Each pre-K classroom has two teachers each
that will break one another therefore keeping each classroom in nap time ratios.
