Inspector Notes
An unannounced monitoring inspection was conducted on 07/25/2022. There were 35 children, 7 months-10 years, and 6 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, physical plant, programs, emergencies and nutrition. A total of 5 children?s records and 5 staff records were reviewed. The children were observed in music and movement, watching television, during story time and having lunch. The inspector discussed the following with the staff: outside play, the arrival of children, rest time and staff interactions. The inspector arrived for the inspection at 9:30 am and departed at 12:00 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
4Evidence: The record for Staff 2 contained documentation that Staff 2 had resided outside of the state of Virginia within the preceding five years. The record for Staff 2 contained a documented hire date of 12/12/2019. There was not a criminal record check from the state in which the individual had resided in the preceding five years available for review during the 07/25/2022 inspection.
Evidence: The record for Staff 5 contained a documented hire date of 05/17/2022. The tuberculosis screening in the record for Staff 5 was dated 05/27/2022.
Evidence: There were two uncovered electrical outlets in the Infant Classroom.
Evidence:
1. The record for Child 1 did not contain the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (there was not an address for the first designated person).
2. The record for Child 5 did not contain the following information: the work phone number and place of employment of each parent who has custody (there was not a work phone number provided for the first parent listed); and the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the address for the second designated person was incomplete).
