Inspector Notes

An unannounced monitoring inspection was initiated on 04/09/2025 and completed on 05/07/2025 with a local agency, in response to a self-report received by the licensing office on 04/07/2025 relating to programs. The inspector reviewed 4 children?s records and 2 staff records on-site.

This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (05/14/2025). A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-40-E
The center shall be in compliance with the center?s own policies and procedures that are required by the standards.

The center?s required discipline policy states that roughly handling a child is not permitted and that punishment by another child is not acceptable. On 04/04/2025, two staff roughly handled multiple preschool children while in their care and allowed punishment by another child.
Plan of Correction: Reiterate to staff about proper handling of the children; retraining.
Standard 8VAC20-780-400-B
SYSTEMIC VIOLATION

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On 04/04/2025, two children had a physical altercation. Instead of redirecting the children, Staff 1 held one child to allow the other child to physically retaliate.
Plan of Correction: Retrain on redirection of negative behavior.

SYSTEMIC VIOLATION

The following actions or threats thereof are forbidden: roughly handling or shaking a child.

On 04/04/2025, the following forbidden actions occurred with children age 4 years old over approximately 10 minutes: Staff 1 pushed Child 3 backwards by striking the child in the forehead. Staff 1 swatted at Child 4, then struck the child in the head and then pulled the child?s hair. Child 4 rubbed their head in the area where their hair was pulled as they walked away. Staff 1 grabbed Child 2 and pushed the child?s head down by the back of their head. Staff 1 then wrapped their arm around the child?s neck, placing the child in a headlock. Staff 1 pushed Child 2 backwards by striking the child in the forehead. Staff 2 roughly handled Child 1 when they jerked the child by their arm multiple times.
Plan of Correction: Employee has been released. Communication with staff and redirection procedures.

SYSTEMIC VIOLATION

The following actions or threats thereof are forbidden: punishment by another child.

Staff 1 held Child 3 (age 4 years) by their shoulders and allowed Child 2 (age 4 years) to strike Child 3 in the face three times.
Plan of Correction: Through retraining and reminding of how to properly deal with children and give them other choices for redirection.
